Who We Are

Hello and welcome to Yellow Bridge — an NDIS Registered Provider dedicated to supporting people with disabilities across Canberra and beyond.

Founded by Muhammad Rahman, a passionate advocate for inclusion and empowerment, Yellow Bridge was born from both professional experience and personal understanding. Muhammad has been serving the Canberra community since 2010 through roles in Care and Protection, Youth Justice, and Mental Health, where he developed a deep appreciation for the resilience and potential of individuals facing complex challenges.

His journey began in 2010 as a Disability Support Worker, supporting people living with dementia. In 2014, he transitioned into Support Coordination, specialising in psychosocial disability and complex support needs. With a Bachelor of Science in Psychology (Honours) and over a decade of hands-on experience, Muhammad brings together clinical insight, compassion, and creativity to help participants achieve meaningful independence and wellbeing.

At Yellow Bridge, we take a strengths-based approach, working collaboratively with participants, families, and providers to find innovative solutions that blend mainstream, natural, and funded supports. Our focus is on outcomes — helping people live independently, confidently, and as active members of their communities.

Muhammad has supported the Canberra community since 2010 across Care and Protection, Youth Justice, and Mental Health. Drawing from both professional and lived experience, he brings empathy and creativity to his leadership at Yellow Bridge.
