Coordination
We connect participants with the supports, providers, and community networks that enable them to build independence and achieve their goals.
Our Coordination Services help participants to gain an understanding of their NDIS plan, and strengthen their ability to connect with the broader systems of supports. They also help participants build and maintain a resilient network of formal and informal supports by giving them the tools and resources they need to not only take charge of their services, but of their lives as well. Our services enable participants to manage all aspects of their plan administration, including engaging service providers, developing service agreements, maintaining service records, paying providers, and claiming payments.
Our Coordination services aim to:
- Improve each participant’s independence and minimise dependence on paid support services
- Help participants develop healthy and meaningful relationships
- Increase the capacity of participants’ natural networks
- Foster community inclusion through individualised and culturally appropriate supports
- Empower participants to take back control and make informed decisions about their lives and support programs
- Develop long-term, more sustainable alternatives to congregated/ segregated support options
We achieve these goals by:
- Gaining a deep understanding of each participant’s unique needs and goals
- Developing individualised goals and plans for each participant
- Identifying natural, unpaid relationships in participant’s lives
- Finding alternatives to minimise reliance on paid support
- Increasing opportunities for meaningful relationships
- Fostering community inclusion
Psychosocial Recovery Coaching
Our Recovery Coaches support individuals living with psychosocial disabilities to strengthen resilience, self-management, and community connections.
Our Psychosocial Recovery Coaching services provide assistance for participants to develop capacity and build resilience through strong and respectful relationships, enabling them to support those with psychosocial disability to live a full and meaningful life.
Our services are designed to give participants the ability to continue engagement through periods of increased support needs due to the episodic nature of mental illness.
Our professional Recovery Coaches work jointly with participants, families, carers and other service providers to identify, plan, design and coordinate tailored NDIS support that best meets the participant’s needs.
